Provide acute and chronic disease management within a patient-centered medical home model, utilizing evidence-based practices.
Coordinate care across various settings, including hospitals, ERs, and nursing facilities, ensuring appropriate discharges and follow-up.
Engage in community outreach to identify and support high-risk individuals.
Qualifications include an unrestricted Ohio medical license, board certification in Internal Medicine or Family Practice, BLS certification, and experience with EMR systems. Preferred candidates have experience in population health and complex patient populations.
