This full-time clinical role focuses on managing complex, chronic illnesses within a comprehensive, community-centered care model.
It involves leading a multidisciplinary team to develop and implement individualized care plans, coordinate transitions across care settings, and treat acute exacerbations in a dedicated clinic setting.
The position requires a full Ohio medical license, board certification in Internal Medicine or Family Practice, and BLS certification. Ideal candidates have experience with population health, complex patients, and EMR systems, with a mission-driven approach to serving underserved populations.
Benefits include a salary of $250,000–$275,000, 401(k) matching, health, life, vision insurance, paid time off, and malpractice coverage. The schedule is Monday to Friday, in person, with no nights or weekends.
