Seeking a Primary Care Physician for an innovative PACE program serving individuals 55+ with chronic illnesses or disabilities to promote independence.

Responsibilities:

Provide high-quality primary care in coordination with an interdisciplinary team

Perform assessments and develop individualized care plans

Manage chronic and acute conditions, deliver preventive care

Participate in care planning, team meetings, and policy development

Make home and facility visits as needed and support staff clinically

Communicate effectively with team members for seamless care

Qualifications include MD/DO, board eligibility or certification in Internal Medicine, Family Practice, or Geriatrics, licensure, and experience with elderly populations.
