Primary Care Physician (MD/DO) – Skilled Nursing Facility Focus

Job Description

Seeking a dedicated MD or DO Primary Care Physician to provide holistic, coordinated care as part of a PACE program.

Responsibilities include:

• Providing medical oversight for Participants in skilled nursing facilities, managing short-term and long-term residents.
• Collaborating with interdisciplinary teams to develop care plans and ensure smooth transitions.
• Conducting assessments for new Participants and transitioning ongoing care to their primary providers.
• Participating in daily team meetings and on-call rotations to support care continuity.

Ideal candidates are mission-driven, collaborative, and experienced in geriatrics or complex care. Requirements include active PA medical license, board certification in Internal or Family Medicine, and strong communication skills.
