Job Description

Seeking a licensed, board-certified Psychiatric Provider to deliver collaborative, population-based mental health care for medically complex and socially vulnerable patients.

Responsibilities include managing behavioral health for a designated cohort through a modified Collaborative Care Model, providing direct patient assessments and tele-psychiatry, supporting medication management (including LAIs and medication-assisted treatment), and collaborating with interdisciplinary teams to improve outcomes.

Candidate must have strong communication skills, experience working with complex Medicaid populations, and a passion for serving underserved communities. This in-office role involves clinical guidance, outreach, and supporting transitions from inpatient care, with an emphasis on integrated, holistic treatment.
