SLP – Speech Language Pathologist

Job Description

The Speech Language Pathologist in Home Health provides assessment and therapy services to residents in a skilled nursing setting, developing personalized treatment plans to meet individual needs.

Conducts screenings and evaluations, reviews medical records, and determines appropriate interventions.

Monitors patient progress, adjusts treatment plans accordingly, and supervises therapy assistants.

Develops programs to maintain and improve residents’ abilities within their environment.

Reviews service quality for effectiveness and efficiency.
