Job Description
The Speech Language Pathologist in Home Health provides assessment and treatment services to residents in a skilled nursing setting.
Conducts screenings, evaluates patient needs, and develops personalized therapy programs to meet individual goals. Reviews physician referrals and medical records to determine treatment necessity. Monitors and adjusts therapy interventions to ensure effectiveness and progress. Supervises licensed assistants and ensures quality of services delivered. Creates programs to maintain and enhance residents’ performance in their environment. Participates in regular assessments and referral processes per facility procedures.
