Job Description

Seeking a dedicated Master Social Worker (MSW) to join an interdisciplinary care team providing comprehensive services to older adults in a PACE program.

  • Conduct in-home assessments and develop care plans.
  • Manage community resources and collaborate with families.
  • Act as an expert in Advance Care Planning and caregiver education.
  • Coordinate care to support seniors living in the community.

    • Requirements include a licensed MSW in Michigan, at least one year of experience with the elderly or long-term care, strong communication and organizational skills, and the ability to work in varied environments.
