Seeking a dedicated Master Social Worker (MSW) to join an interdisciplinary care team providing comprehensive services to older adults in a PACE setting.
Conduct in-home assessments, develop care plans, and manage community resources.
Collaborate with families and facilitate meetings to improve quality of life.
Serve as an expert in Advance Care Planning and provide caregiver education.
Must hold an MSW, be licensed in Michigan, and have at least one year of experience with the elderly or long-term care populations.
Require strong communication, organizational skills, and proficiency in MS Office.
