Job Description

Seeking a dedicated Master Social Worker (MSW) to join an interdisciplinary team providing comprehensive care for older adults in a community-based program.

Conduct in-home assessments and develop personalized care plans.

Manage community resources and collaborate with families.

Serve as an expert in Advance Care Planning and caregiver education.

Coordinate services to support aging in place and improve quality of life.

Requirements: MSW licensed in Michigan, at least one year of experience with the elderly or long-term care, strong communication, organizational skills, and proficiency in MS Office. Must meet vaccination and competency standards.
