Social Worker (LMSW or LLMSW)

Job Description

We are seeking a dedicated Master Social Worker (MSW) to join our interdisciplinary care team providing comprehensive services to older adults in a community-based setting.

Conduct in-home assessments and develop personalized care plans.

Manage community resources and collaborate with families.

Serve as an expert in Advance Care Planning and caregiver education.

Maintain confidentiality, demonstrate compassion, and support team goals.

Requirements include a Master’s Degree in Social Work, licensure in Michigan, at least one year of experience with the elderly or long-term care population, and strong communication skills. Candidates must be fully vaccinated against certain illnesses and meet CMS standards.
