Job Description
Seeking a dedicated Speech Language Pathologist (SLP) to join our in-house therapy team.
Responsibilities:
Evaluate, diagnose, and create individualized treatment plans for residents with speech, language, cognitive-communication, and swallowing disorders.
Provide evidence-based therapy services per physician orders and facility policies.
Collaborate with an interdisciplinary team including therapists, nurses, and physicians.
Educate patients, families, and caregivers on treatment goals and home programs.
Maintain accurate clinical documentation in compliance with regulations.
Participate in care planning and discharge meetings.
Qualifications:
Master’s Degree in Speech-Language Pathology from an accredited program.
Current State Licensure or eligibility, and CCC-SLP from ASHA (CFY candidates welcome).
Experience in a skilled nursing facility preferred but not required.
Strong communication and organizational skills.
