Responsibilities include communicating with office staff, accepting new patients, completing evaluations, plans of care, reassessments, and discharge paperwork, as well as attending meetings and case conferences.

Requirements include current licensure, a master’s or doctorate degree, ability to travel within a 40-mile radius, strong communication skills, and home healthcare experience is preferred.
