Speech Language Pathologist – $5,000 BONUS

Job Description

We are seeking a Speech Language Pathologist (SLP) to join our home health team, providing skilled therapy services in patients’ homes.

• Deliver care per state guidelines, patient plans, and interdisciplinary team coordination.
• Evaluate patients’ abilities and develop personalized treatment plans for communication and swallowing disorders.
• Document sessions, educate patients and families, and adjust care as needed.
• Participate in team meetings, maintain accurate records, and provide occasional in-services.
