Speech-Language Pathologist-Home Health

Job Description

We are seeking a Home Health Speech-Language Pathologist (SLP) to provide in-home speech, language, cognitive, and swallowing therapy.

Conduct comprehensive assessments and develop personalized care plans aligned with physician orders and patient goals.

Implement evidence-based treatments, educate patients and families, and adapt plans based on progress.

Ensure compliance with CMS Conditions of Participation, state and federal regulations, and accurate documentation.

Collaborate with interdisciplinary team members to coordinate care and optimize outcomes.

Provide patient and caregiver education on communication and swallowing strategies.

Participate in quality improvement initiatives and maintain timely documentation.
