Job Description
The Home Health Speech-Language Pathologist (SLP) provides evidence-based speech therapy services in patients’ homes, evaluating and treating speech, language, cognitive, and swallowing disorders.
They conduct assessments, develop individualized care plans, and implement therapeutic interventions while educating patients and caregivers to enhance communication and swallowing abilities.
The role ensures compliance with CMS Conditions of Participation, federal and state regulations, and maintains accurate documentation.
Collaborating with an interdisciplinary team, the SLP participates in case conferences and quality improvement initiatives to optimize patient outcomes.
Requirements include a valid Ohio licensure. The position involves travel, flexible hours, and may include evening and weekend visits.
