Job Description

This position provides in-home speech therapy services to patients in the San Jose area, focusing on speech, language, cognitive-communication, and swallowing disorders.

  • Conduct initial evaluations and develop personalized care plans.
  • Deliver therapy, monitor progress, and reassess regularly.
  • Document patient outcomes and communicate with the care team and physicians.
  • Collaborate with interdisciplinary teams and educate patients and caregivers to promote independence.
  • Complete required documentation, including OASIS when applicable.

    • Qualified candidates are licensed Speech-Language Pathologists with a Master’s degree, at least one year of experience, and a compassionate, patient-focused approach.
