Job Description
This role provides in-home speech therapy services to patients in the San Jose area, focusing on speech, language, cognitive-communication, and swallowing disorders.
Perform initial evaluations, develop individualized care plans, and deliver skilled therapy sessions. Reassess patient progress regularly, document outcomes, and communicate with the care team and physicians. Educate patients and caregivers to promote independence and positive health outcomes. Collaborate with an interdisciplinary team to coordinate care. Require a Master’s degree, active state licensure, and at least one year of experience as a Speech-Language Pathologist.
