Job Description
This role involves providing specialized clinical care as a Speech-Language Pathologist (SLP) for patients with communication disorders in a home health setting.
Assess client needs through thorough history and physical exams, document findings within 24 hours, and interpret assessment data holistically. Develop and update individualized care plans with measurable goals, deliver safe, competent interventions, and coordinate care with clients and their families. Communicate with insurance providers to determine eligibility and assist in obtaining payment sources. Provide client and caregiver education, promote self-care, and ensure safety and infection control standards are met. Supervise and evaluate staff, participate in team and quality review meetings, and support new staff orientation. Maintain compliance with licensure, certifications, documentation requirements, and safety protocols. Require a Master’s in Speech-Language Pathology, valid license, and relevant clinical experience.
