Job Description
We are seeking an experienced and reliable home care Speech Language Pathologist to conduct home visits and provide speech therapy services based on physician-approved plans of care.
Assess and evaluate patients’ speech and language abilities, developing and updating treatment plans with measurable goals.
Provide rehabilitative speech and language therapy using appropriate techniques and materials.
Document patient progress, prepare assessments, discharge summaries, and participate in care plan revisions.
Collaborate with healthcare team members, participate in conferences, staff meetings, and quality improvement initiatives.
Instruct patients, caregivers, and other healthcare personnel on home therapy programs.
Ensure compliance with regulations, report violations, and perform other duties as assigned.
Requirements include a master’s degree, ASHA certification, licensure in New York, and preferred experience in institutional and home care settings.
