Job Description

The role involves assessing referred patients requiring rehabilitation services and providing direct patient care to evaluate their medical condition, functional abilities, and limitations.

  • Develop and implement personalized treatment plans with achievable goals based on patient capacity and physician guidance.
  • Document patient progress, conduct evaluations, and prepare progress notes, recertifications, and discharge summaries.
  • Educate patients and families on therapy methods.
  • Collaborate with multidisciplinary teams to deliver patient-centered care.
  • Perform additional duties as assigned by the Director of Rehabilitation, ensuring compliance with confidentiality, HIPAA, and residents’ rights.

    • Qualifications include graduation from an accredited Speech Therapy program, valid NY state license, and registration. Experience in long-term or geriatric care is preferred.
