Job Description

Seeking a PER DIEM Speech Therapist for a Skilled Nursing Facility in Rochester, NY, at Wesley Gardens.

Provide quality SLP services, including patient evaluation, screening, referrals, data collection, and plan of care implementation.

Complete timely documentation such as progress notes, screen reports, daily notes, and discharge summaries.

Offer expertise in short-term and long-term SLP care, and instruct patients, families, and staff on goals and therapeutic techniques.

Ensure compliance with protocols and regulations.

Qualifications: Valid NYS License & Certificate of Clinical Competency, experience with cognitive and dysphagia rehabilitation, preferred knowledge of Rehab Optima.

Compensation: $53.00 – $58.00 per hour.

Requirements: Ability to commute and relocate to Rochester before starting.
