Job Description
We are seeking a Per-Diem Speech-Language Pathologist (SLP) to join a 24-hour, 90-bed skilled nursing facility in Long Beach, CA.
Provide comprehensive patient care, including evaluations, treatments, care plans, family education, and documentation.
Communicate patient status effectively with patients, families, and healthcare team members.
Initiate evaluations based on physician orders and develop treatment plans aligned with reimbursement guidelines for optimal outcomes.
Educate staff and third-party payers on intervention benefits and outcomes.
Complete accurate documentation such as evaluations, progress notes, and reports.
Screen individuals for therapeutic needs and ensure safety at all times.
Collaborate with interdisciplinary teams to develop programs and protocols.
Utilize electronic medical records and rehab systems for documentation.
Qualifications include a degree in Speech and Language Pathology, active license, CPR/BLS certification, and preferred experience in skilled nursing facilities.
