Job Description

We are seeking a compassionate and organized Per Diem Licensed Speech Therapist (ST) to join a leading home health agency serving the Santa Clara County area.

• Work collaboratively with a multidisciplinary team to provide coordinated patient care.
• Deliver speech therapy services within the scope of practice, following individualized care plans.
• Evaluate patient responses, report changes, and maintain accurate clinical documentation.
• Follow all organizational policies and procedures.

Requirements:
