Job Description

We are seeking a dedicated Full-Time Nurse Practitioner or Physician Assistant to join our Wound Care/Home Health team in Centralia, WA.

  • Provide wound evaluations and management during weekly bedside rounds at skilled nursing facilities, including dressing changes, debridement, and G-Tube management (training provided).
  • Perform Home Health visits as needed.
  • Work independently within facility protocols, establishing care plans for acute and chronic wounds.

    The ideal candidate is outgoing, energetic, and experienced in wound care and skilled nursing environments.

  • Requirements include 2+ years in wound care, skilled nursing, and debridement, current Washington license, and strong communication skills.

    Compensation ranges from $125,000 to $180,000 annually, with benefits such as health/dental insurance, paid time off, and flexible scheduling.

    Schedule is Monday-Friday, day shift, with on-call and weekend duties as needed.
