Job Description

We are hiring a full-time Nurse Practitioner or Physician Assistant to join our Wound Care/Home Health team in Centralia, WA.

Provide wound evaluation and management during weekly bedside rounds in skilled nursing facilities.

Perform dressing changes, medically necessary debridement, and G-Tube replacements (training provided).

Conduct Home Health visits as needed.

Work independently within facility protocols and collaborate with treatment teams.

The ideal candidate is outgoing, energetic, and experienced in wound care and skilled nursing environments.

Requirements include at least 2 years of wound care and SNF experience, strong communication skills, current WA license, and wound care certification preferred.
