We are expanding our mobile wound care team to serve more patients in Cumberland and Franklin Counties, due to growth and a successful care model.

Role Responsibilities

  • Conduct assessments and develop personalized treatment plans
  • Provide bedside wound care in skilled nursing and assisted living facilities
  • Lead wound assessments, treatments, and procedures
  • Utilize advanced technology and evidence-based practices to promote healing
  • Collaborate with facility teams and families for holistic care decisions
  • Maintain timely documentation in our EMR system
  • Support quality initiatives and patient-centered care improvements

Qualifications

  • Active PA Nurse Practitioner license or Physician Assistant license
  • Prescriptive authority and DEA license (must obtain within training period)
  • Minimum 2 years wound care experience
  • Experience in SNFs, long-term care, or rehab preferred
  • Willingness to travel within Cumberland/Franklin Counties
  • Comfort with EMR and mobile documentation
  • Strong communication and teamwork skills
