We are expanding our mobile wound care team to serve more patients in Cumberland and Franklin Counties, due to growth and a successful care model.

Role Responsibilities
- Conduct assessments and develop personalized treatment plans
- Provide bedside wound care in skilled nursing and assisted living facilities
- Lead wound assessments, treatments, and procedures
- Utilize advanced technology and evidence-based practices to promote healing
- Collaborate with facility teams and families for holistic care decisions
- Maintain timely documentation in our EMR system
- Support quality initiatives and patient-centered care improvements
Qualifications
- Active PA Nurse Practitioner license or Physician Assistant license
- Prescriptive authority and DEA license (must obtain within training period)
- Minimum 2 years wound care experience
- Experience in SNFs, long-term care, or rehab preferred
- Willingness to travel within Cumberland/Franklin Counties
- Comfort with EMR and mobile documentation
- Strong communication and teamwork skills
