Job Description
This full-time position offers a flexible, dynamic environment focused on improving patient outcomes across diverse populations, primarily in Iowa. It involves travel with mileage reimbursement and allows for 2 days of remote work weekly.
Provide comprehensive wellness exams and physical assessments for geriatric patients.
Manage chronic and acute conditions in various healthcare settings, including nursing homes and assisted living facilities.
Utilize EMR/EHR systems such as Epic, Cerner, and others for accurate documentation and coding.
Offer health coaching, patient education, and follow-up care to promote wellness and disease prevention.
Collaborate with interdisciplinary teams on case management and post-acute care planning.
Proven clinical experience with diverse populations, strong EMR proficiency, and knowledge of hospice or palliative care is preferred.
The role includes benefits like 401(k), health and dental insurance, paid time off, and a competitive salary range of $125,000–$135,000.
