The Nursing Home Wound Care Advanced Practitioner (AP) is a full-time clinical professional, such as a Nurse Practitioner or Physician Assistant with specialized wound management training, responsible for resident wound assessment, treatment, and prevention in a PALTC facility.

They aim to reduce complications, improve care quality, enhance patient and family experience, decrease hospitalizations, and lower costs.

Key duties include comprehensive assessments, individualized care planning, performing advanced wound treatments, collaborating with interdisciplinary teams, developing prevention protocols, staff education, documentation, quality improvement, and resident advocacy.

They ensure regulatory compliance, manage infections, and engage residents and families in wound care education. Strong communication, leadership, and technology skills are essential.

Qualifications include a relevant degree, at least 2 years of experience, board certification (AANP/ANCC/PA-C), and preferred specialty certification in wound care.
