Job Description
The Nursing Home Wound Care Advanced Practitioner (AP) is a licensed Nurse Practitioner or Physician Assistant with specialized wound management training, responsible for assessing, treating, and preventing wounds in long-term care residents.
Perform comprehensive wound assessments and develop individualized care plans. Administer advanced wound treatments such as debridement and negative pressure therapy. Collaborate with healthcare teams to ensure integrated wound management and prevention strategies. Educate staff, residents, and families on best practices and wound care protocols. Maintain detailed documentation and participate in quality improvement activities. Address infections, ensure regulatory compliance, and advocate for resident needs. Provide leadership and communication within interdisciplinary teams, utilizing technology tools effectively. Qualifications include a relevant degree, 2+ years of experience, board certification, and preferred specialty certification in wound care. The role is full-time with benefits, working both in person and remotely as needed.
