Job Description

The Certified Nurse Practitioner in the Nursing Home serves as a key member of the Institutional Interdisciplinary Care Team, providing
care management, clinical assessments, and education to members, families, and staff.

Responsibilities include triaging care needs using HRAT scores, conducting ongoing assessments, facilitating access to services,
evaluating patients post-discharge, and advocating for self-management.

This full-time role involves visiting 1-2 nursing homes daily, ensuring timely diagnosis and intervention with a Monday-Friday schedule, 8am-4:30pm, offering a great work/life balance.

Requirements include a valid Ohio Nurse Practitioner license, at least 3 years of healthcare experience, preferably in HMO/PPO settings.
