Job Description
We are seeking a compassionate Family Practice Provider to deliver comprehensive primary care in a dynamic setting, with plans to assume this practice in a year.
Provide preventive care, diagnose illnesses, manage chronic conditions, and coordinate treatment plans for patients of all ages. Conduct physical exams, develop individualized treatment strategies, and administer injections, vaccines, and urgent care as needed. Utilize EHR systems like Epic or eClinicalWorks for documentation and coordinate referrals to specialists or hospital services. Engage in patient education, health coaching, and participate in clinical research when applicable. Requirements include a valid medical license, family or internal medicine certification (board certification preferred), experience with EMR systems, and strong clinical skills.
