Job Description

Help the neediest communities by providing in-home healthcare as a Nurse Practitioner, focusing on high-risk Medicaid and AHCCCS members. You will assess patients, develop treatment plans, prescribe medications, and educate families to promote better health outcomes. Collaborating with a team of medical professionals, you’ll manage a caseload of about 100 members, visiting five patients daily to ensure progress and address barriers. Your role includes coordinating care, overseeing transitional procedures, and engaging patients around their goals.

Requirements include valid licensure as a Nurse Practitioner, experience in primary or acute care, excellent communication skills, and a compassionate approach. The position covers the Yavapai East area, including several communities, and offers a starting salary of $130,000 with comprehensive benefits, bonuses, and opportunities for professional growth. Bilingual in Spanish is preferred.
