Job Description

Assist high-risk, high-need Medicaid and AHCCCS patients in their homes to improve health outcomes through care coordination, education, and treatment planning.


  • Conduct patient exams, review records, and create integrated treatment plans.
  • Prescribe and reconcile medications, and educate patients and families.
  • Collaborate with healthcare providers and team members to ensure comprehensive care.
  • Manage a panel of about 100 members, meeting with each monthly to track progress and goals.
  • Oversee transitional care and address care gaps to enhance patient engagement and success.

    • Requires licensure as a Nurse Practitioner in Arizona, with experience in primary, acute, or chronic care. Strong communication, teamwork, and patient connection skills are essential. The role covers the Yavapai East area, with a starting salary of $130,000 plus benefits, bonuses, and comprehensive insurance options.
