Job Description

We are hiring a Part-Time LCSW Geriatric Psych Social Worker for long-term care facilities in Manitowoc, WI.

Provide direct psycho-social services, including evaluations, diagnosis, and treatments such as individual, group, and family therapies to nursing home residents.

Collaborate with an interdisciplinary team to ensure quality care and continuity, adhering to Medicare guidelines and documentation standards.

Manage a flexible caseload of 10-15 patients daily, with opportunities to adjust based on scheduling needs.

Enjoy autonomy in facilities with clinical support, modern electronic charting, and a dedicated practice manager.

Qualifications: LCSW with a master’s degree, 1-2 years’ experience, full state licensure, strong communication skills, and proficiency with technology.
