Job Description
We are hiring a part-time LCSW with geriatric behavioral health experience for a clinical social worker role in Downers Grove, IL.
Provide psycho-social services, including evaluations, diagnosis, and treatment for nursing home residents.
Conduct individual, group, and family therapies, seeing 10-15 patients daily with flexible scheduling.
Collaborate with interdisciplinary teams to ensure quality care and continuity.
Maintain knowledge of Medicare guidelines and proper documentation of clinical contacts.
Qualifications include an active State license, Master’s degree, 1-2 years’ experience, and proficiency with technology.
Preferred experience in healthcare/nursing home settings and with Medicare standards.
