Job Description

We are seeking a full-time Social Worker (CSW / LCSW) to support social service programs in a healthcare facility. The role involves planning, developing, and evaluating programs, coordinating care, discharge planning, and connecting residents and families to community resources.

  • Participate in community planning and assist with resident assessments and care plan development.
  • Engage residents and families in program planning and arrange transportation as needed.
  • Refer residents to appropriate social service agencies.

    • Requirements include a Bachelor’s degree in Social Work or a related field, at least 2 years of experience in a nursing facility, and an active CSW or LCSW license. Strong interpersonal and communication skills are essential.
