Seeking a Licensed Social Worker to join a dedicated hospice team serving the Charleston, SC area.
In this role, you will
interact effectively with diverse individuals conduct social assessments including family dynamics and caregiver abilities identify high-risk cases for issues like suicide, neglect, or abuse develop intervention plans based on evaluation findings collaborate as part of the interdisciplinary care team Qualifications include
