We are seeking a Full-Time Nurse Practitioner (NP) for our Post-Acute Care team in Sumter, SC. This Monday–Friday, 8:00 AM–4:00 PM role offers clinical autonomy and a supportive, team-based environment.

  • In-person care at local Post-Acute & Skilled Nursing Facilities
  • Collaborative care with physicians and on-site teams
  • Salary: $100,000–$120,000/year plus performance bonus
  • Benefits include health, dental, vision, 401(k), PTO, CME stipend, and paid holidays


    • Qualifications: Active SC NP license, board certification, strong clinical skills, and a passion for geriatric and chronic care. Experience in post-acute or outpatient settings preferred.
