Job Description

We are seeking an experienced Diabetic Practice Nurse to join a friendly, city-centre GP surgery supporting 13,800 patients. The role involves delivering structured diabetes reviews, optimizing glycaemic control, initiating and titrating medications, providing patient education, and working with the wider MDT to maintain high-quality care and achieve QOF targets. This part-time position (23-30 hours/week) requires flexibility to work across two sites in Chichester and Bosham, with duties including face-to-face and phone consultations.

Applicants must be NMC-registered, have primary care diabetes experience, hold a relevant qualification, and be confident with insulin management.
