Job Description
We are seeking an experienced Diabetic Practice Nurse to join a friendly, city-centre GP surgery due to retirement.
Deliver structured diabetes reviews, optimize glycaemic control, and manage medications including injectables. Provide patient education, promote self-management, and support quality improvement initiatives. Work 23-30 hours per week, including Mondays, across two sites in Chichester and Bosham. Must be a registered nurse with NMC registration, have primary care diabetes experience, and hold or be working towards a recognized diabetes qualification. Strong communication skills, organization, and the ability to work independently and as part of a team are essential.
