Primary Care Physician (MD/DO) – Skilled Nursing Facility Focus

Job Description

Seeking a dedicated MD or DO Primary Care Physician for a PACE program focused on holistic, coordinated senior care.

As a Skilled Nursing Facility (SNF) Physician, you will oversee medical care for Participants in six SNFs, manage acute and chronic conditions, and coordinate with interdisciplinary teams to ensure seamless care transitions and reduce hospitalizations.

You will also conduct enrollment assessments, develop initial care plans, and facilitate ongoing care transfer to primary providers.

Responsibilities include rounding, managing patient needs, participating in daily team meetings, and providing on-call coverage (approx. 1:6 weeknights/weekends).

Ideal candidates are mission-driven, collaborative, and experienced in geriatrics or SNF management, with active PA medical licensure, board certification, and strong communication skills.
