Job Description

We are seeking a qualified Nurse Practitioner (Skilled Nursing) to join our team in a Skilled Nursing Facility (SNF) setting.

  • Serve as a clinical leader, collaborating with the care team to oversee short- and long-term residents.
  • Perform admissions, discharges, monthly stability checks, and evaluate changes in residents’ conditions.
  • Handle advanced care planning, comprehensive assessments, and prescribe treatments.
