We are seeking a qualified Nurse Practitioner (Skilled Nursing) to join our team in a Skilled Nursing Facility setting.

Serve as a clinical leader by collaborating with the care team to oversee residents’ short- and long-term needs.

Perform admissions, discharges, monthly stability checks, and evaluate changes in condition.

Handle advanced care planning, comprehensive assessments, and prescribe treatments.

Requirements:

Active Pennsylvania APN licensure and DEA.

Previous nursing or clinical experience.

Strong communication and teamwork skills.

Master’s degree in Nursing.
